• Crossed cheque with “Account Payee Only”
•  Payable to Uni.Asia Life Assurance Berhad
•  The policy number must be written on the reverse of the cheque
•  Mail cheque together with the premium payment statement to the address indicated at the bottom of the front page of Premium Payment Statement.
