• Complete the deposit slip with the following information :-
  - Nama Pembayar : (Your Name)
  - Nama Penerima : Uni.Asia Life Assurance Berhad
  - Account No. : 14100-29-86450015-5
  - No. Rujukan/No. Polisi : eg. IL0000E34200-5
  - Amaun Bayaran : Payment Amount
