- Special arrangements have been made with most major hospitals and medical centres. For your convenience, no admission deposit is required for all pre-authorized hospital admission (subject to terms and conditions).
- Dedicated hotline to provide you assistance on admission to a hospital.
High Lifetime limit
Lifetime limit at 12 times your annual limit.

| Benefits of Smart Health |

* Health Bonus (HB), equivalent to 10% of Insurance Charges for Smart Health in the preceding policy year, will be credited to the Account Value if no medical claim is made in the preceding policy year. This benefit will only be available after 3 policy years have been completed. At the end of the 3rd policy year, an amount equal to 10% of the total insurance charge over the first 3 years will be credited, provided that no claims have been made until that time.

| Who Is Eligible To Apply? |
Anyone between the age of 30 days and 65 years old may apply for Smart Health. Coverage can be renewed up to age 87 years old.

| What Are The Risks That Are Not Covered? |
Smart Health does not cover any of the benefits if hospitalization, surgery, or medical services is directly or indirectly, wholly or partly from or due to:
- Pre-existing conditions;
- Specified illnesses occurring during the first 120 days of the continuous cover;
- Any medical or physical conditions arising within the first 30 days of the insurance coverage commencement date or any date of reinstatement whichever is the latest except for accidental injuries;
- Plastic/Cosmetic surgery, circumcision, eye examination, glasses and refraction or surgical correction of nearsightedness (Radial Keratotomy or Lasik) and the use or acquisition of external prosthetic appliances or devices such as artificial limbs, hearing aids, implanted pacemakers and prescriptions thereof;
- Dental conditions including dental treatment or oral surgery except as necessitated by Accidental Injuries to sound natural teeth occurring wholly while this Rider is in force;
- Private nursing, rest cures or sanitaria care, illegal drugs, intoxication, sterilization, venereal disease and its sequelae, AIDS (Acquired Immune Deficiency Syndrome) or ARC (AIDS Related Complex) and HIV related diseases, and any communicable diseases required quarantine by law;
- Any treatment or surgical operation for congenital abnormalities or deformities including hereditary conditions;
- Pregnancy, child birth (including surgical delivery), miscarriage, abortion and prenatal or postnatal care and surgical, mechanical or chemical contraceptive methods of birth control or treatment pertaining to infertility. Erectile dysfunction and tests or treatment related to impotence or sterilization;
- Hospitalization primarily for investigatory purposes, diagnosis, X-ray examination, general physical or medical examinations, not incidental to treatment or diagnosis of a covered Disability or any treatment which is not Medically Necessary and any preventive treatments, preventive medicines or examinations carried out by a Physician, and treatments specifically for weight reduction or gain;
- Suicide, attempted suicide or intentionally self-inflicted injury while sane or insane;
- War or any act of war, declared or undeclared, criminal or terrorist activities, active duty in any armed forces, direct participation in strikes, riots and civil commotion or insurrection;
- Ionising radiation or contamination by radioactivity from any nuclear fuel or nuclear waste from process of nuclear fission or from any nuclear weapons material;
- Expenses incurred for donation of any body organ by the Life Assured and costs of acquisition of the organ including all costs incurred by the donor during organ transplant and its complications;
- Investigation and treatment of sleep and snoring disorders, hormone replacement therapy and alternative therapy such as treatment, medical service or supplies, including but not limited to chiropractic services, acupuncture, acupressure, reflexology, bonesetting, herbalist treatment, massage or aroma therapy or other alternative treatment;
- Care or treatment for which payment is not required or to the extent which is payable by any other insurance or indemnity covering Life Assured and Disabilities arising out of duties of employment or profession that is covered under a Workman’s Compensation Insurance Contract;
- Psychotic, mental or nervous disorders, (including any neuroses and their physiological or psychosomatic manifestations);
- Costs/expenses of services of a non-medical nature, such as television, telephones, telex services, radios or similar facilities, admission kit/pack and other ineligible non-medical items;
- Sickness or Injury arising from racing of any kind (except foot racing), hazardous sports such as but not limited to skydiving, water skiing, underwater activities requiring breathing apparatus, winter sports, professional sports and illegal activities;
- Private flying other than as a fare-paying passenger in any commercial scheduled airlines licensed to carry passengers over established routes;
- Expenses incurred for sex changes.

| • Some Important Things You Need To Know: |
- You may cancel Smart Health within 15 days from the delivery date of the policy contract by returning it to Us. We will refund any Cost of Insurance charged.
- This plan may qualify you for the personal tax relief under education and medical insurance up to RM3,000, subject to the final decision of the Inland Revenue Board.
- In the event of claims, you have to notify Us within 30 days of completion of the events for which the claim is being made.
- The Cost of Insurance of this rider is NOT GUARANTEED and may be varied by the Company at the Policy Anniversary date. You will be given 3 months prior notice of any such variation.
- If You stay in a higher Room & Board, then all eligible expenses will be subjected to 20% co-insurance, up to a maximum cap of RM1,000 per disability.
- Please be aware that there may be implications that will affect your health insurance application if you are switching from one type of health plan to another or from another insurer to Uni.Asia Life Assurance Berhad.
- If you elect to or are referred to be treated outside Malaysia by the Attending Physician, Reasonable and Customary charges will be reimbursed. Payment will be subject to the lower of the actual charges or the charges for similar treatment carried out in hospitals in Malaysia. However, the cost of transport to the place of treatment is excluded.
- No benefit will be payable for any medical treatment you receive outside Malaysia if you reside or travel outside of Malaysia for more than 90 consecutive days.

- The Company reserves the right to amend the terms and provisions of this rider by giving a 30 day prior notice in writing and such amendment will be applicable from the next renewal of this rider.
